Elevate Basketball & Jump Squad

Our purpose is to provide kids in Kindergarten - 6th grade basic sports instruction in an effort to develop them physically, mentally, and spiritually. 

Games and practices are held weekly at Hoops at 12101 Sycamore Station Place Louisville, Kentucky 40299.  All practices will be on Monday evenings and games will be on Friday evenings.  First practice is Monday, November 28th.  First game is Friday, January 6th.  Last game is on Friday, February 24th.

WAIVER

By registering, I agree that Northeast Christian Church will not be held liable for any injury incurred while my child participates in 2011-2012 Elevate Basketball and/or Jump Squad.  I will be responsible for all medical costs except those covered by my insurance.  I authorize the director of these activities and/or his/her choice and treatment of my child on an emergency basis if necessary.  I acknowledge the fact that this child is physically able to participate in all activities.I give permission to Northeast to use photographs of my child for publicity and/or on their website.
